Job description

The Health Systems Specialist 2 (HSS 2) will work with the interdisciplinary team in the Center for Hospice and Palliative Care to ensure that research, policy, and training includes an integrated approach to patient-centered care. Targeting both health care providers as well as patients and families, the HSS 2 will be a crucial staff member and will work to discover deficiencies, improve communication, and increase education around hospice and palliative care issues, with subject matter expertise on advance care planning conversations and tools, bereavement support, and care coordination community resources.

Minimum Qualifications

Permanent Transfer Candidates: Current NYS Department of Health employee with one year of permanent or contingent-permanent, competitive, status as a Health Systems Specialist 2 (Social Work), G20 OR current NYS employee with one year of permanent or contingent-permanent, competitive, status in a title G18 or higher and eligible to transfer under Section 70.1 of the NYS Civil Service Law.

This title is part of the New York Hiring for Emergency Limited Placement Statewide Program (NY HELPS).

Non-Competitive Candidates Via NY HELPS: Possession of New York State license and current registration as a Licensed Master Social Worker AND one year of post-licensure experience providing clinical services in a licensed healthcare facility or program or utilizing clinical knowledge in the review and oversight of clinical services for quality assurance or reimbursement purposes; or registration as a Licensed Clinical Social Worker.

Preferred Qualifications: The preferred candidate will have strong time-management, critical-thinking and problem-solving skills. The candidate will be able to communicate effectively with both internal and external stakeholders and have strong MS Office skills. Candidates with experience in Hospice, Palliative Care, Grief Counselling, Advance Care Planning preferred, due to nature of program.
