The Licensed Master Social Worker will serve as a member of a multidisciplinary treatment team. They will participate in team meetings, present clinical information and observations, and provide consultation and assistance to staff in other disciplines. The incumbent will coordinate care and treatment planning for individuals and assess/ evaluate individual needs, strengths, and limitations; records significant psychosocial events and/or changes in clinical conditions. The Licensed Master Social Worker advocates for individuals’ rights and access to benefits and services to meet their personal and social needs. They will help individuals identify goals and solutions to a variety of short-term mental, emotional, behavioral, and environmental conditions. They will use therapy to address illnesses and disorders, including mental illnesses and emotional disturbances, marital and family difficulties, adjustment problems related to acute and chronic illnesses, addiction, substance use, behavioral, learning disorders, and community problems and social issues.
The Licensed Master Social Worker will establish and maintain professional relationships with community providers to ensure continuity of care, while assisting residents with the development and maintenance of natural supports. They will link residents and families to existing services in the community to ensure continuity of care. The Licensed Master Social Worker will develop effective treatment plans in conjunction with residents and treatment teams. They will monitor and evaluate residents’ progress and modify treatment plans or terminate services when appropriate. The incumbent will determine discharge service needs and appropriate community resources, while signing off on finalized discharge service documents and following up with community providers to ensure residents are successfully integrated into the community. They will prepare and maintain documentation and records, including progress notes, observations, recommendations, and discharge plans. Under the supervision of a licensed clinical social worker, licensed psychologist, or psychiatrist, they may provide clinical services to individuals, such as the diagnosis of mental, emotional, behavioral, developmental, and addictive disorders; development of assessment-based treatment plans; and provision of psychotherapy.
Current New York State employee with one or more years of permanent, contingent-permanent, or 55 b/c service in a position allocated to Grade 18 or above, and that is eligible for transfer under Section 70.1 of the Civil Service Law to Licensed Master Social Worker.
If less than three interested and qualified New York State Department of Health employees serving in the direct-line, lower-level title(s) within the agency apply to this opportunity, the agency may make a non-competitive appointment under the NY HELPS program.
Open Competitive HELPS qualifications: Possession of a license and current registration as a Licensed Master Social Worker in New York State.
Preferred Qualifications: The preferred candidate will have long term care experience with a geriatric population
