Job description

Incident Monitoring & Quality Review

  • Provide real-time, statewide monitoring and oversight of the incident management function, ensuring serious reportable incidents, abuse allegations, and deaths are accurately classified, thoroughly documented, and addressed with immediate protective measures and required notifications.
  • Review completed investigations and supporting documentation submitted by providers; identify deficiencies, request additional evidence or investigative work, and ensure accurate data entry into the Incident Report and Management Application (IRMA) and the Vulnerable Persons' Central Register (VPCR).
  • Review and track Corrective Action Plans (CAPs), follow up with providers to obtain required information, and update IRMA and VPCR systems accordingly.
  • Analyze IRMA data to identify trends and generate Critical and Significant Event reports.

Administrative Operations

  • Serve as liaison to the NYS Justice Center, responding to inquiries and coordinating requests for additional information regarding investigations and CAPs.
  • Respond to inquiries from the Justice Center, voluntary and state-operated agencies, and unit colleagues regarding incident management protocols and procedures.

Technical Assistance, Training & Collaboration

  • Provide ongoing training, technical assistance, and system support to state-operated and voluntary provider agencies on incident management protocols and the use of IRMA.
  • Collaborate with internal and external partners --including the Office of Investigations and Internal Affairs (OIIA), and local committees --to support a coordinated response to serious incidents.
  • Coordinate with Division of Quality Improvement (DQI) units (BPC and CQI) to share monitoring data and escalate significant untoward events for immediate follow-up.
  • Participate in local and statewide committees, work groups, and meetings, to contribute to OPWDD incident management initiatives.

Compliance & Technical Proficiency

  • Maintain a comprehensive understanding of 14 NYCRR Part 624 regulations, the Protection of People with Special Needs Act (PPSNA), and IMU administrative processes to provide accurate advice and direction.
  • Maintain proficiency in Microsoft Office Suite, OPWDD and NYS Justice Center databases, and all internal Incident Management Unit (IMU) systems.
  • Perform other duties as assigned.

Minimum Qualifications

Open Competitive:

Program Operations Specialist Trainee 1, NS (equivalent to SG-14):

A bachelor’s degree in a human services field.

Program Operations Specialist Trainee 2, NS (equivalent to SG-16):

A bachelor’s degree in a human services field and one year of experience in a federal or state certified human service program, which must have been in a professional administrative position in which a major duty involved planning, coordinating, monitoring or evaluating a provision of services for people with developmental disabilities. This experience must have included information gathering and analysis.

Program Operations Specialist 1, SG-18:

A bachelor’s degree in a human services field and two years of experience in a federal or state certified human service program. One year of this experience must have been in a professional administrative position in which a major duty involved planning, coordinating, monitoring or evaluating a provision of services for people with developmental disabilities. This experience must have included information gathering and analysis.

Competitive:

Candidates must be reachable for appointment on the current Civil Service Eligible List or the Professional Career Opportunities (PCO) Eligible List for this title.

Substitution:

A Juris Doctor, master's or higher degree in a related field can substitute for one year of specialized experience.

Transfer:

Candidates must have one year of permanent competitive or 55-b/c status in the posted title and be eligible for lateral transfer; OR be eligible to transfer via Section 70.1, 70.4, or 52.6 of Civil Service Law. The transfer cannot be a second consecutive transfer resulting in an advancement of more than two salary grades.
