Job description

The Registered Nurse 2 (Community) will be a member of the interdisciplinary treatment team for Westchester County. The RN2 will provide clinical oversight and coordinate nursing services for individuals in Hudson Valley DDSOO's community facilities and programs. The position:

  • Provides, monitors, and coordinates nursing and health related care services for a caseload of individuals in a community setting in Hudson Valley's medically enhanced homes and Article 16 outpatient clinic sites (main and satellite locations).
  • Performs/monitors and oversees the clinical case management for individuals that are medically frail, and/or that are dually diagnosed.
  • Serves as a member of an interdisciplinary treatment team and provides nursing-specific clinical, consultative, and instructional assistance to individuals and their families, other disciplines and health care providers, and the Article 16 clinic facilities.
  • Monitors and coordinates the care that may be needed for individuals who are admitted into a hospital/nursing home setting.
  • Monitors/coordinates admissions and discharge planning activities and liaises with community providers, families, hospitals, and other health care facilities.
  • Acts as a liaison to facilitate care and treatment for individuals through the Article16 main and satellite clinic sites.

This RN2 will provide clinical oversight and coordinate nursing services for individuals who reside within one of the medically enhanced homes in Hudson Valley and work closely with the Article 16 outpatient clinic to ensure appropriate services are available and provided to individuals in HV. This position is critical to maintain appropriate clinical supervision and support in the site and to ensure that health and safety is maintained for medically complex individuals served as the safety net provider.

This is one of several enhanced nursing sites within HVDDSOO that requires 24 hour nursing coverage by both LPNs and RNs.

Minimum Qualifications

Applicants must possess a license and current registration as a registered professional nurse in New York State; and two years of post-licensure clinical nursing experience. One year of this experience must have been providing direct services to individuals diagnosed with mental illness, developmental disabilities, addiction, or substance use disorders in a community setting.

Substitution:

A bachelor's degree in nursing may substitute for one year of general post-licensure clinical nursing experience; and a master's degree in nursing may substitute for two years of general post-licensure clinical nursing experience.

Transfer:

Candidates must have one year of permanent competitive or 55-b/c status in the posted title and be eligible for lateral transfer; OR be eligible to transfer via Section 70.1, 70.4, or 52.6 of Civil Service Law. The transfer cannot be a second consecutive transfer resulting in an advancement of more than two salary grades.
